Welcome to the Official APS Withdrawal Form
Please complete a separate withdrawal form for each student. This form is only intended for families moving out of the Atlanta Public Schools (APS) District. If you are relocating within the APS District to a different attendance zone, please begin the enrollment process at your new APS school.
Important Information – Please Read Before Continuing:
To officially withdraw a student from the APS District, an Official Withdrawal Request Form must be completed for each student. Upon submitting this form, please ensure the student returns all of the following items by the last day of school:
* Textbooks
* Library books
* Uniforms
* Any school-issued equipment or devices
Device Return Information - All APS-issued devices must be returned to the student's school. If the school is closed or staff are unavailable, devices can be returned to the APS Central Office at 130 Trinity Avenue, Atlanta, GA 30303.
APS Board reserves the right to charge students a reasonable fee for the restitution of lost, damaged, or abused property, as specified in this policy. Students shall be notified in writing of any charges assigned to them, and, that failure to pay the fee may result in sanctions until restitution is made. Opportunity shall be given for the student and/or parent to meet with the appropriate school official to discuss any fines or fees levied against the student. Requirements for Withdrawal:
* Only the parent or legal guardian who originally enrolled the student may request a withdrawal.
* A copy of a valid driver's license or state-issued ID is required to begin the process.
* You must provide your new address.
* Please include the name of the school your student will be transferring to. If it is out of state, include the name of the state as well.
* If you intend to homeschool your child, complete the Georgia Department of Education Homeschool Declaration of Intent form. Please download the completed form and have it ready to upload when submitting your withdrawal request.
Once the withdrawal is verified and completed, you will receive a confirmation email with instructions for the new school to request student records. APS does not accept records requests via fax, email, or in person.
If you do not have an email address, please contact your child’s school directly to schedule an in-person withdrawal appointment.
